A student who is officially enrolled in the College Unit is issued an identification (ID) card by the Student Affairs Office (SAO) which is validated at the same office during the registration and enrollment period. Important information regarding the ID card and its use are as follows:

1.  The ID card is required upon entering the campus and must stay pinned to the blouse’s/shirt left collar or worn with SSC/blue ID lace while inside the campus.

2.  Tampering with or changing the photo invalidates the ID card and is considered a major offense.

3.  Lending of the ID card to others is strictly prohibited and is a major offense.

4.  A student whose ID card is confiscated shall render disciplinary sanction within one week from the commission of the offense before she/he can redeem her/his ID card. While her/his ID is confiscated, she/he shall present the blue form signed by the Dean of Student Affairs (DSA) at Gate 1 and to her/his teachers/class representative.

5.  A student who forgets/loses her/his ID shall present two (2) letters stating the reason for her/his not having her/his ID at Gate 1. The guards-on-duty shall stamp both letters and forward one copy to the SAO. The second letter serves as a one-day pass. A student who habitually forgets/loses her/his ID (at least three times) shall be meted disciplinary sanctions.

6.  A student who loses her/his ID card is required to submit an Affidavit of Loss certified by a Notary Public to the SAO and pay a replacement fee to the Business Office. While the replacement ID is being processed, the affidavit of loss duly signed by the DSA shall be presented at Gate 1 and to her teachers/class representative. A student should be able to secure a new ID within three days from its reported loss.
